Le service dispose de 30 places.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading has-vivid-cyan-blue-color has-text-color has-link-color wp-elements-2\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-bleu-color\">La prise en charge<\/mark><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:var(--wp--preset--spacing--20);margin-bottom:var(--wp--preset--spacing--20);padding-top:0;padding-bottom:0\">Vous devez avoir une prescription m\u00e9dicale de votre m\u00e9decin traitant pour que le SAD puisse intervenir.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:var(--wp--preset--spacing--20);margin-bottom:var(--wp--preset--spacing--20);padding-top:0;padding-bottom:0\">Les interventions \u00e0 domicile se font g\u00e9n\u00e9ralement entre 8h et 12h et entre 17h et 20h.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:var(--wp--preset--spacing--20);margin-bottom:var(--wp--preset--spacing--20);padding-top:0;padding-bottom:0\">Le service est financ\u00e9 par l&rsquo;Assurance Maladie et les usagers n&rsquo;ont pas de reste \u00e0 charge.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading has-text-color\" style=\"color:#68d9f0\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-bleu-color\">L\u2019\u00e9quipe<\/mark><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019\u00e9quipe est dirig\u00e9e par une infirmi\u00e8re coordinatrice et compos\u00e9e d\u2019aides soignantes.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading has-text-color has-link-color wp-elements-3\" style=\"color:#64d6ed\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-bleu-color\">La zone d\u2019intervention<\/mark><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Le service peut intervenir&nbsp;dans les communes de :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Garlan<\/li>\n\n\n\n<li>Guimaec<\/li>\n\n\n\n<li>Lanmeur<\/li>\n\n\n\n<li>Locquirec<\/li>\n\n\n\n<li>Plouegat-Guerrand<\/li>\n\n\n\n<li>Plouezoc\u2019h<\/li>\n\n\n\n<li>Plougasnou<\/li>\n\n\n\n<li>Saint-Jean-du-Doigt.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading has-text-color has-link-color wp-elements-4\" style=\"color:#64d6ed\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-bleu-color\">Les soins dispens\u00e9s<\/mark><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:var(--wp--preset--spacing--20);margin-bottom:var(--wp--preset--spacing--20);padding-top:0;padding-bottom:0\">Le SAD peut dispenser :<\/p>\n\n\n\n<ul style=\"margin-top:0;margin-bottom:0;padding-top:0;padding-bottom:0\" class=\"wp-block-list\">\n<li>des soins infirmiers qui sont r\u00e9alis\u00e9s par des infirmiers lib\u00e9raux (convention avec le service)<\/li>\n\n\n\n<li style=\"margin-top:0;margin-bottom:0;padding-top:0;padding-bottom:0\">des soins d&rsquo;hygi\u00e8ne et de confort<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Il met \u00e9galement en place une surveillance de l&rsquo;\u00e9tat de sant\u00e9 g\u00e9n\u00e9ral de la personne (poids, pouls, hydratation, ;..) et veille \u00e0 pr\u00e9venir les complications li\u00e9es \u00e0 l&rsquo;immobilisation (risques d&rsquo;escarres)<\/p>\n\n\n\n<div style=\"height:48px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading has-text-color has-link-color wp-elements-5\" style=\"color:#68d9f0\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"wp-block-paragraph\">Votre admission d\u00e9pendra du nombre de place de disponibles au moment de la demande.<br>SI l&rsquo;intervention est possible, l&rsquo;infirmi\u00e8re coordinatrice :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Evaluera vos besoins dans le cadre d&rsquo;une visite \u00e0 domicile<\/li>\n\n\n\n<li>Elaborera un plan de soins (type de soins et nombre de passages chaque jour ou chaque semaine) <\/li>\n\n\n\n<li>Vous proposera un document individuel de prise en charge. <\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Une fois votre accord donn\u00e9 sur ce
